TOYOTA SIENNAS About 94,000 2011 model year Sienna vehicles produced before early November 2010 to replace the stop lamp (brake light) switch bracket. Due to the proximity of the stop lamp switch bracket to the parking brake pedal when the parking brake is fully applied, the stop lamp switch bracket could, in limited circumstances, be deformed by the driver’s foot during parking brake operation. Depending on the amount of bracket deformation, this condition could cause the brake lights to remain on or, if the deformation is significant, the brakes could become partially engaged due to contact from the bracket, resulting in brake drag with associated brake noise, brake vibration, and/or illumination of the brake warning light. If this condition is not noticed and the vehicle continues to be driven, braking effectiveness could be reduced.
IF YOU OWN ONE Toyota has notified owners of the problem with instructions on what to do. Toyota dealers will replace the bracket assembly, which includes the stop lamp switch bracket, with a new one that has been redesigned to take into account the proximity of the component to the parking brake pedal.
INFANT TEETHERS About 37,000 Refreshing Rings infant teethers/rattles sold at mass merchandise and baby specialty stores nationwide between July 2009 and January 2011 for about $5. Small pieces of the plastic ball can detach as a result of children chewing on the teether/rattle, posing an ingestion hazard. The firm has received one report of pieces of the black plastic from the polka dot ball detaching while being chewed. No injuries have been reported.

CANDY All Toxic Waste Brand Nuclear Sludge candy products distributed nationwide and through mail order from the product's inception in May 2009 through January 2011. Some pieces contain elevated levels of lead (0.101 parts per million to .311 ppm; the U.S. FDA tolerance is 0.1 ppm) that potentially could cause health problems, particularly for infants, small children, and pregnant women. The company has determined to recall all lots and all flavors of the smaller sized product. No illnesses have been reported to date in connection with this problem.
